What “initial each page” actually asks of you
“Please initial each page and sign on the last page” means: write your initials, the first letters of your first and last name, somewhere on every page, then put your full signature in the signature block at the end. It's a request from the other party, not a formality of the law in most cases. Here's what it's for, what it looks like done right, and what's at stake if a page is missed.

Proof you saw every page
Your signature on the last page records that you agreed. Initials on each page record which pages were in front of you.
Protection against swapped pages
Replacing page 7 after signing is easy if page 7 carries no mark. Your initials on it make the swap obvious.
A nudge to read
Stopping on each page to initial it slows people down, which is partly the point. Nobody should be surprised by page 7 later.

- Does “initial each page” include the signature page?
- Usually not, because your full signature is on it. Initial it too if the page has its own initials box or the instructions say every page without exception.
- Do I initial the back of double-sided pages?
- Yes, if the back has content. Each printed side is a page. Blank backs don't need initials.
- Can someone else initial the pages for me?
- Only with proper authority, such as a power of attorney, and then they sign in their own name as your representative. Initials written by someone else pretending to be you are not your initials.
- What does “initial and date each page” mean?
- Your initials plus the date you initialled, on every page. Use the same date format on every page.

The instruction, word by word
“Initial” as a verb means to write your initials. For most people that's two letters, the first letter of the first name and of the last name; some add a middle initial. “Each page” normally means every page of the document, attachments included, apart from the page you sign in full, unless that page has its own initials box.
Variations you may see: “initial all pages” means the same thing; “initial and date each page” asks for the date next to each set of initials; “initial where indicated” means only the marked spots, not every page; and “initial every page including exhibits” spells out that schedules and attachments count.
Is it legally required?
For most everyday contracts, no law demands page initials. A lease, a job offer or a service agreement usually becomes binding when it's signed the way the law and the document itself require, and page initials are an extra safeguard the other side asks for. If they ask and you skip it, expect the document to come back to you.
Some documents are different. In South Africa, the Wills Act requires every page of a will other than the one on which it ends to be signed by the person making it, and it defines signing to include initials, so initialing each page of a will there is a legal formality, not a habit. England and Wales take another route: the Wills Act 1837 asks for the will to be signed and witnessed, and says nothing about initials on each page, although some solicitors ask for them anyway.
What happens if you miss a page
Usually, a missing initial doesn't undo a contract that's otherwise properly signed. What it does is give the other side a reason to send the document back, and in a dispute about what page 7 said, a page without your initials is one less piece of evidence that it was the page you saw.
With a will the stakes are higher. Where the law requires each page to be signed, as in South Africa, an unsigned page can put the will at risk. South African courts can still order a document to be accepted as a will if they are satisfied it was meant to be one, but that means going to court, which is exactly what careful signing avoids.
Where the initials go and how they should look
If the document has an “Initials” box or line, that's the place. Otherwise the bottom right corner is the convention, in the margin rather than over the text. Use the same initials in the same style on every page. Typed initials are normal in electronic documents; handwritten ones are normal on paper.
